**New Jersey State Ombudsman
You can call the State Ombudsman to file a complaint against a licensed long term care facility for resident issues such as:
- Violation of resident’s rights and dignity
- Physical, verbal, or mental abuse
- Deprivation of services
- Poor quality of care including inadequate personal hygiene
- Improper transfer or discharge from the facility
- Inappropriate use of chemical or physical restraints
- Financial exploitation

**Social Security Administration
To schedule an appointment with your local Social Security Office or to request information, call the National Social Security number is 800-772-1213 or visit their website www.ssa.gov (a lot of information can be requested on line but NOT all)

**PAAD (Pharmaceutical Assistance to the Aged & Disabled)
This is the New Jersey State prescription benefits program
Need to be 65 years old or disabled through Social Security to qualify

PAAD Income caps are:
Single – $25,743
Married – $31,563
(They look at your TOTAL annual income only NOT assets)
**SSI Medicaid (Supplemental Security Income)
you file through your local Social Security office. It is better to call the National number and schedule an appointment in order to decrease your wait time at the local office.
Need to be 65 or disabled to qualify for this program.
Income and Asset caps for SSI are:
Single – $741.25 per month in income and $2,000 in assets
Married – $1,091.36 per month in income and $3,000 in assets
**NJ Care Medicaid
Need to be 65 or disabled to qualify for this program
Income and Asset cap are:
Single – $958 per month in income and $4,000 in assets
Married – $1,293 per month in income and $6,000 in assets
**Global Options Medicaid (GO Program)
This program became effective January 2009 and it replaced three of the prior Medicaid Waiver programs. GO participants are eligible for all NJ Title XIX Medicaid services and must receive Care Management services and one additional waiver service such as Assisted Living, Respite Care, Home Based Supportive Care, Environmental Accessibility Adaptations, Personal Emergency Response Systems, Home Delivered Meal Service, Caregiver/Participant Training, Social Adult Day Care, Special Medical Equipment & Supplies, Chore Service, Care Management, Transitional Care Management, Transportation, and Attendant Care.
Need to be 65 or disabled and require nursing home level of care to qualify.
Income and Asset cap are:
Single – $2,130 per month in income and $2,000 in assets
(If married they only look at the applicant’s total income but they count joint assets – each spouse would file individually for this program)
